“Doctor,Is there any emergency or can I wait a little longer to get my cataract surgery?”“Will my eyes get permanently damaged if I delay my cataract surgery for a few weeks or months?”
“My eye doctor has told me that if I don’t get the cataract surgery done urgently, my lens may burst and damage the eye?”
Thesearesome of the most common questions patients ask me when they get diagnosed with cataract for the first time. Sometimes, doctors also misguide patients by putting fear into their mind to get urgent cataract surgery done.
And honestly, the confusion is understandable.
Many people believe cataract surgery should only be done when the cataract is “fully mature or ripe.” Others delay it out of fear. Some think it can be treated with eye drops and chase expensive medical treatments for long without any benefit.
But the truth is different and let’s explore it in an easy to understand way.
As an eye doctor with experience of more than 15000 successful cataract surgeries, let me tell you very honestly-when you actually need cataract surgery and when you can safely wait.
What is a Cataract? Let us understand this first.
A Cataract is when the natural lens of the eye becomes cloudy over time. Just like greying of your hair, the fibres of your lens start becoming whitish-yellow (due to denaturation of lens proteins). As more and more fibres of the lens become yellowish and lose their transparency, your vision starts getting blurred. This usually happens between ages of 50 to 75 but can even happen at 40 or as late as 90. It is a part of natural ageing and develops in everyone, even in animals. The whitishness of the lens or the cataract leads to:
- • Blurred vision- everything becomes hazier than usual (especially at distance)
- • Glare (especially at night) – difficulty in night driving
- • Difficulty reading (in later stages)
- • Faded colors and dullness
It is a natural ageing process—but timing the surgery correctly is very important for best results.
So, what is the right time to get Cataract Surgery done?
It Has A Very Simple Answer:
You need cataract surgery when your vision starts affecting your daily life. Not earlier. Not unnecessarily late. When you can feel a difference in the quality of your vision and your day to day activities start getting affected, there is no benefit of waiting, even though there is no emergency for surgery either.
Signs that it’s the right time:
- • You are unable to read clearly even with glasses
- • Night driving becomes difficult due to glare
- • You struggle with mobile or TV and reading the finer prints
- • Frequent change in glasses power but vision does not improve completely
- • Vision is affecting your work or independence
Cataract should become fully mature before surgery
This was true 20–30 years ago when cataract surgery was done with a large incision and had higher complication rates.
Today, modern cataract surgery is safer when done earlier or moderate stage, not late. Waiting too long can make surgery more complicated and has affect final outcomes.
Eye drops can cure cataract
Patients keep wasting their money trying different ayurvedic treatments and eventually come back to get cataract surgery done. Truth is, no scientifically proven eye drops can reverse a cataract. Surgery is the only effective treatment.
Cataract surgery is risky
Modern techniques like phacoemulsification and femto-laser assisted cataract surgery (FLACS) make cataract surgery one of the safest procedures in medicine, with very high success rates reaching close to 99.9%.
I can manage with glasses, so no need for surgery
While if your vision is good with glasses, you can continue with them. But, if your quality of life is affected, delaying surgery only prolongs discomfort.
Old age means I should avoid surgery
Age is not a barrier. Overall health and eye condition matters more than age. I have even operated a 101 years old for cataract and the patient was very happy after his clear vision got restored.
What Happens If You Delay Cataract Surgery Too Much?
Delaying unnecessarily can lead to:
- • Harder cataract (more complex surgery)- and higher chances of complications
- • Increased risk of complications as the lens becomes difficult to break with the phaco machine
- • More chances of errors in lens power calculations- with advanced cataracts, calculations of the intraocular lens that is to be implanted in your eye after cataract removal, may become inaccurate resulting in imperfect outcomes.
- • Rise in eye pressure – can happen rarely if surgery is prolonged for very long
- • Prolonged and delayed post-op visual recovery
Waiting too long doesn’t make surgery better - it often makes it harder. While patients always ask for safe surgery and perfect outcomes, as an eye doctor with 15 years of experience and more than 15000 successful cataract surgeries, I can tell you honestly that surgery becomes much more challenging for surgeons to perform once the cataract is becoming hard and mature.
Is Cataract Surgery Urgent?
In most cases, cataract surgery is planned, not emergency, not even an urgency. So if your cataract is in early or moderate stage – you can consider delaying it for a few weeks to a few months after discussing with your doctor. But if your vision is affected, you don’t gain anything by delaying the surgery, it will be needed eventually. Early surgery will restore your visual clarity faster and relieve you from your day to day vision difficulties. If you have a family wedding to attend or a vacation that you had planned or a family matter to take care of, you can easily delay the surgery to get it sorted out and then get yourself operated.
However, cataract surgery may become urgent if:
- • Vision suddenly worsens
- • Cataract becomes very dense
- • There is associated increased eye pressure or pain
Cataract Surgery in Diabetic Patients
We have high number of patients coming for cataract surgery who also have diabetes. Patients with diabetes need special attention. However, cataract surgery can be safely performed in these patients with no significant major risks, if right precautions are followed.
